First Doctor Visit: Tailbone Pain
He has been having exquisite tailbone type pain for about a year and a half now.

X-Ray Sacrum + Coccyx
TYPE OF EXAM: Views of the sacrum and coccyx. FINDINGS: There is no fracture of the sacrum. There is volar angulation of the coccyx.

Osteopathic Exam
There is an increased lumbar lordosis, TART changes to the lumbar spine. L5 is rotated R.

Diagnosis
Consideration was given as to wether manipulative therapy could provide improvement or worsening of function.

Second Doctor Visit: Knee Pain
The patient reports that there was no trauma and the pain is mainly located under the kneecap.

Review of Systems
He just bought a knee brace that he has been using for the past couple of days. No hip pain or ankle pain.

Diagnosis
I told the patient it does look like he may have tendinitis in his knee.

Third Doctor Visit: Subjective
Patient reports breaking his coccyx a couple years ago. Patient says he has 8-9 years of skateboarding and playing lacrosse and his LBP/sacral pain started gradually after breaking his coccyx.

Gait Diagnosis
Abnormal–Decreased pelvic rotation.

Accessory Mobility
There is hypomobility noted at the L4-L5 vertebrae and extending to the left sacroiliac joint (L SIJ)

Palpation
TTP (Tender to Palpation) is noted on the right (Ⓡ) at specific areas: • There is also tenderness in the PSIS on the right. • Severe hip flexor (HF) tightness is noted on the right. • Bilateral tightness of the piriformis and tightness in the ITB on the right side.

Finding My True Path
